Procedures for Issuance of Passport:

Passport Renewal:
- 2 passport pictures
- Old passport
- Nafl.150 persons ages 13 to 59
- Nafl.130 persons ages 12 and under & 60 and older

First time Passport application:
- Birth certificate (foreign birth certicates have to be legalized
 or apostille
- 2 passport pictures
- Nafl.150 persons ages 13 to 59
- Nafl.130 persons ages 12 and under & 60 and older

Passport application for Naturalized/Option persons:
- Naturalization/Option Certificate
- Foreign Passport
- Birth certificate (foreign birth certificates have to be legalized
 or apostille, translated to Dutch or English
- 2 passport picture
- Nafl. 150 persons ages 13 to 59
- Nafl. 130 persons ages 12 and under & 60 and older

Lost/Stolen:
- Police Report
- Other form of Identification
- 2 passport Pictures
- Nafl. 270 persons ages 13 to 59
- Nafl. 250 persons ages 12 and under & 60 and older

- All minors age 0-17 must be accompanied by parents/guardian.
- Both parents must be present if they are married.
- If parents are divorced, parent with guardianship must be present
with guardianship declaration.

Pick up procedure:
- Applicant must be present (including minors 0-17) with a parent or guardian.
 - Receipt
- Old passport in cases of renewal application

Please note that there is a new implementation of fingerprinting for all
applicants from 12 years and older.
