Requirements
• Valid identification card or passport of informant
• Doctors declaration of death (brown and white envelopes which must remain sealed)
• Dutch nationals need to submit identification cards and passport for perforating
• A legalized birth / marriage certificate of spouse if not registered at the Civil Registry Department
• Declaration from Prosecutor’s Office (for official releasing of the corpse in the event of an investigation))
• Declaration from the Government appointed examiner approving the death and the transport of the corpse
• Fee: NAFL 25.00 for death certificate
• Fee: NAF 25.00 for Apostille Stamp (if death certificate will be used abroad)
Notices
• Payment must be made at the begin of process in cash or maestro only
• In some instances additional information may be requested from the informant in order to proceed with the death registration
Once these documents have been submitted and the process completed, the Civil Registrar will give a death certificate and permission for burial or cremation.
